  • Bullying and Harassment Complaint Form

    Click the link above to complete the form and report an incident of alleged bullying or harassment. Completed forms submitted for review are monitored Monday-Friday during regularly scheduled office hours.

    After recording your submission, a staff member will contact you to assist with completing a more thorough report. 

    By submitting a completed form, you acknowledge:
    - answers provided in this report will be shared and discussed with the appropriate school site staff.
    - designated CVUSD staff will contact the Complainant to discuss this complaint.

    *PLEASE NOTE: All submissions will be kept confidential to the fullest extent possible, but some information may be revealed as necessary to conduct an effective investigation. Be advised, if a submission is submitted anonymously, the District may be limited in its ability to conduct a thorough investigation.

    The Chino Valley Unified School District seeks to provide a safe school environment that allows all students equal access and opportunities in the District's academic, extracurricular, and other educational support programs, services, and activities. Students are encouraged to report any form of bullying, discrimination, or harassment immediately.

    The Chino Valley Unified School District recognizes the harmful effects of bullying on student learning and school attendance and desires to provide a safe school environment that protects students from physical and emotional harm while promoting mutual respect, tolerance, and acceptance.

    Any person that has been a victim of, or has witnessed bullying or harassment is highly encouraged to report the incident immediately to a school counselor, an administrator, or another adult personnel on campus.
